Overview:
Many organizations have embraced population health management as the path to delivering better health outcomes at lower cost. For many, however, translating that to action is still a work in process. Adopting a new business model has far-reaching implications for quality and cost management, payment mechanisms, and relationships with physicians. Based on his client work and Numerof’s National Survey of the State of Population Health, Michael Abrams highlights the roadmap to population health, and the challenges involved in implementing a value-based care delivery model.
