Overview:
Many organizations have embraced population health management as the path to delivering better health outcomes at lower cost.  For many, however, translating that to action is still a work in process. Adopting a new business model has far-reaching implications for quality and cost management, payment mechanisms, and relationships with physicians. Based on his client work and Numerof’s National Survey of the State of Population Health, Michael Abrams highlights the roadmap to population health, and the challenges involved in implementing a value-based care delivery model.

Learning Objectives:
At the completion of this webinar, the participants will be able to:

  • identify why population health is gaining traction as a way to increase health outcomes and reduce costs
  • articulate the seven steps required to get to population health
  • anticipate common implementation challenges and how to overcome them

Target Audience:
This webinar targets executive leadership, service line management and others with operational responsibility in making population health work in the organization.

Non-member rate: $400 per connection

Thursday, May 12, 2016.  Registrations made after this date will incur a $25.00 late fee.
Registration includes one internet connection and one telephone connection at one location and an unlimited number of participants from your organization in one listening room.

A confirmation notice and instructions on how to access the Webinar, along with any handout materials, will be emailed a couple days prior to the event.
