AzPHA Annual Award Nominations
Each year the AzPHA recognizes persons who’ve made an outstanding contribution to public health in Arizona. Our awards are named after people who have made career-long important contributions to public health and/or the AZ Public Health Association over the years. You can visit our AzPHA website to see our past awardees.
Please take some time to nominate a colleague who has made a special contribution to public health! We have awards in several categories. Below are summaries of the awards and links to the nominating forms.
We will be recognizing our nominees and award winners at a recognition event associated with our 2019 Annual Meeting.
The 2019 Senator Andy Nichols Honor Award honors those who have made outstanding contributions to public health policy. The award is named after the late Senator Andy Nichols, an advocate for good public health policy.
The Cele Cohen Nursing Award was established for the purpose of recognizing the contributions of Cecelia Cohen to public health nursing. This award is to recognize a public/community health nurse within Arizona that has made an outstanding contribution to public/community health.
The Harold B Woodward Award is given for work done for the advancement or betterment of the Arizona Public Health Association. Hard work, dedication, or any other contribution to the Association will be the guideline for the selection. Nominees must be members of the Arizona Public Health Association.
The Elsie Eyer CUP Award creates an opportunity to recognize persons or entities that have demonstrated a sustained effort in assuring the delivery of health services to underserved populations in Arizona.
Underserved people are those who face barriers to accessing services because they have difficulty paying for services, have language or cultural barriers, have insufficient resources in the community, have disparities in health status, are homeless, or are at-risk school children.
The Lloyd E. Burton Scholarship is intended to attract students to careers that will improve the public’s health. The scholarship is in recognition of the late Lloyd E. Burton, Ph.D. for his sustained support of AzPHA and his efforts to encourage interest in professions that improve the public’s health.
1. Applicants should be Arizona students who are currently enrolled in a degree-seeking program related to public health (e.g. nursing, medicine, public health, health administration, etc.).
2. Applicants may not be current employees of the Arizona Public Health Association
3. Students must also be current members of AZPHA (Student memberships are $25 and you may become a member online at www.azpha.org)
2019 Health Education Media Making Yearly Awards
Any health education or patient education material produced in Arizona by an Arizona resident(s) during August 2018 –August 2019 is eligible for a HEMMY award. No item may be entered in more than one category. Categories are for Professional Submissions, Grassroots Submissions, and for a Student (any category)
If the same materials have been submitted for a nomination in a previous year the submitted forfeits a refund and the materials will not be reviewed. The materials must be newly created each year for submission.
There will be awards for each category: First Place: Plaque* Second Place: Certificates*
Winners will be recognized at our Annual Conference on Tuesday October 29th, 2019 Please email an image of your submission to [email protected]. If your award wins one of the 3 prizes it will be showcased during this portion.

The first of this series will take a closer look at options for Standard Direct Contracting Entities (DCEs). Subsequent webinars will examine New Entrant and High Needs Population DCEs. Attendees will hear from policy and data experts as well as from newly forming DCEs. Participants should come away with a better understanding of the financial ramifications of Direct Contracting participation.
