Clinical Evidence, Payer Requirements, and Arizona RHTP Opportunities
Learning Objectives
  1. Identify evidence-supported RPM applications and associated clinical outcomes for hypertension, type 2 diabetes, and heart failure.
  2. Identify the clinical and operational processes needed to translate remotely collected physiologic data into timely clinical interventions that support improved patient outcomes, while recognizing how Medicare and AHCCCS requirements shape RPM service delivery.
  3. Evaluate whether RPM is appropriate for a rural healthcare organization based on clinical need, patient suitability, operational readiness, and rural implementation barriers.

Course Description

Remote patient monitoring has the potential to improve clinical outcomes by extending patient monitoring beyond traditional healthcare encounters. However, its effectiveness depends on selecting appropriate patients and using remotely collected physiologic data to support timely clinical interventions. Rural healthcare organizations must also understand how payer requirements shape service delivery and establish the workflows, technology, and care team responsibilities needed to operate an effective RPM program.

This webinar will examine evidence-supported RPM applications and associated clinical outcomes for hypertension, type 2 diabetes, and heart failure. Participants will explore the clinical and operational processes needed to translate remotely collected data into clinical action, review how current Medicare and Arizona Medicaid requirements influence RPM service delivery and consider the factors that determine whether RPM is appropriate for their organization. The webinar will also discuss how RPM may align with Arizona Rural Health Transformation Program priorities and opportunities.

About the Presenter

Carrie Foote serves as the President-Elect of the Arizona Rural Health Association. She joined the Board of Directors in January 2024 as Secretary and was elected to her current role in July 2025. In her work with AzRHA, Carrie supports statewide efforts to strengthen partnerships, education, and policy initiatives that improve the health of Arizona’s rural communities. She focuses on advancing practical, sustainable solutions that help rural communities thrive.

Carrie is a healthcare strategy consultant with more than a decade of experience designing and optimizing telehealth programs. She strengthens telehealth access in rural communities and supports organizations in launching and expanding telehealth service lines through comprehensive program planning, design, and implementation. Carrie partners with health systems, clinics, clinicians, and public agencies across the country to build effective and resilient virtual care strategies. She is the co-founder and principal consultant of Foote Forward Health Strategies.
