To make sure AHCCCS providers have time to participate in the program, the Arizona Medicaid agency pushed its application deadline to August 18, 2017.
The program, which The Arizona Health Care Cost Containment System (AHCCCS), calls “one-time opportunity to support integrated and coordinated care for AHCCCS members” is part of the agency’s strategy to incentivize integrated care of the state’s Medicaid beneficiaries.
Targeted Investment Program Goals:
- Reduce fragmentation between acute and behavioral healthcare
- Increase efficiencies in service delivery for members with behavioral health needs
- Improve health outcomes for the affected populations.
All providers interested in applying in the program are eligible if they use an Electronic Health Record to exchange electronic health data without “special effort on part of the user.” Providers must also have completed a behavioral health integration assessment provided by AHCCCS.
The program is open to Arizona primary care providers, behavioral health providers and hospitals. Learn more from AHCCCS
