No Place Like Home Phase 2 Kick Off Event

In January 2012, Arizona kicked off the No Place Like Home statewide campaign and within 18 months Arizona grandly exceeded its goal and made the greatest improvement in readmission reduction in the nation!

Now, in 2015, Arizona will kick off Phase 2 of the No Place Like Home Campaign to reduce readmissions by focusing on effective medication management.

Some say medication management and all that comes with it (medication reconciliation, adherence, affordability, etc.), is too huge a problem to fix, another impossible mission. We’ve heard that before, and we are ready to say “Mission: Possible!”

Join us on March 23, 2015, and learn about the key goals for No Place Like Home Phase 2 and how to participate. National and Arizona best practices will be profiled, campaign goals will be explained, and experts will be available to share their journey with you.
