Primary Business / ServicesCommonwealth Primary Care ACO (CPCACO), is one of the nation’s few primary care founded, owned, operated and governed Accountable Care Organization (ACO).
As an ACO, we have contractual relationships with payors (including Medicare and commercial insurance) and other health care providers (including specialists, hospitals, home health agencies and post-acute care facilities) to lower overall health care costs while improving overall health care quality and patient satisfaction. Our emphasis is to return the primary care practice and the patients they serve to the focal point of healthcare decision-making. We offer the support and flexibility for providers to remain in independent practice while being able to provide coordinated care for their patients.
As primary care physicians we know what you need and are committed to your practice and the patients you serve. We have no beds to fill, no shareholders to serve and no egos to stroke. We are dedicated to delivering the highest quality medical care, in a most efficient manner, as we help primary care physicians achieve financial success, and therefore the solvency of the independent primary care practice.
Our membership base consists of over two hundred primary care physicians and over one hundred additional Nurse Practitioner and Physician Assistant midlevel providers. The CPCACO is designated as a Medicare Shared Savings Program (MSSP) ACO as chartered by the Center for Medicare/Medicaid Services (CMS) and is registered to do business throughout the State of Arizona and New Mexico, although we currently do not have any New Mexico physicians. CPCACO has additional contracts with commercial and Medicare Advantage payor organizations, and is always seeking to expand into value-based contracts. Some specific examples are CIGNA for over 8,000 commercial lives and United HealthCare Commercial with approximately 13,000+ additional commercial lives. CPCACO intends to pursue additional payor arrangements for all lines of business.
